About this template

Clinics need a working email address before they send test results or reminders through a patient portal. This confirmation email for Clearpath Clinic, a sample practice, asks for one click and explains why in plain words.

A teal shield with a check mark sets a safe, calm mood. The heading and one sentence ask the patient to confirm, and a rounded button does the job. A short section called Why we ask lists what the portal will send. A muted privacy note says what the email will never contain.

Many patients are older or not used to portals, so keep the text large and the words simple. Give the clinic phone number for people who prefer to call. Do not include any health details in this first email.

Use it in your email tool

  1. Click Copy HTML, or download the .html file.
  2. In Mailchimp, Brevo, HubSpot or Klaviyo, start a new email with the custom HTML or code editor and paste it in.
  3. Change the text, links and images. Map merge tags such as {{first_name}} and {{unsubscribe_url}} to the tags of your tool.
  4. Send a test to yourself on desktop and phone before you send it to your list.
