About this template

Small online shops can send this survey a week after an order arrives. It suits stationery, gift, book and craft stores that want honest feedback without a long form. Light mint and soft green keep it fresh and friendly.

The shop name sits at the top with a small drawing of an envelope and a pencil. One question asks how the order went, and a short line explains why the answer matters. The rating row shows the numbers 1 to 5 as large links, each one going to its own answer page, with a small key under it. A thank you code for 10% off the next order closes the email.

One click is all you ask, so more people answer. Point each number at a page that saves the score and then asks one optional follow-up question.

Use it in your email tool

  1. Click Copy HTML, or download the .html file.
  2. In Mailchimp, Brevo, HubSpot or Klaviyo, start a new email with the custom HTML or code editor and paste it in.
  3. Change the text, links and images. Map merge tags such as {{first_name}} and {{unsubscribe_url}} to the tags of your tool.
  4. Send a test to yourself on desktop and phone before you send it to your list.
