About this template

A short survey that asks one question: how likely is the reader to recommend the product to a friend. It is made for software and service companies that track a simple score after a few weeks of use.

A serif wordmark sits on the light grey top. A teal band holds the question as a big white headline, one line that explains the scale and a photo of a laptop on a desk. Under the band the answers are round orange buttons from 0 to 10, set in three short rows so they stay easy to tap on a phone, with a small note that explains what the ends of the scale mean.

A few lines thank the reader and say how the answers are used, and the team signs off. A dark footer band holds the social links, the address and the unsubscribe link.

The layout follows a classic open source survey template. Point each button at a link that records the score, then show a thank you page with one optional question.

Use it in your email tool

  1. Click Copy HTML, or download the .html file.
  2. In Mailchimp, Brevo, HubSpot or Klaviyo, start a new email with the custom HTML or code editor and paste it in.
  3. Change the text, links and images. Map merge tags such as {{first_name}} and {{unsubscribe_url}} to the tags of your tool.
  4. Send a test to yourself on desktop and phone before you send it to your list.
